- a statement that you have a good-faith belief the use is not authorized by the rights holder, its agent, or the law;
- a statement, under penalty of perjury, that the information is accurate and that you are the rights holder or authorized to act on their behalf;
- your physical or electronic signature.

3. Our Response
We may remove or disable access to material we believe in good faith is infringing, notify the affected user, and terminate the accounts of repeat infringers where appropriate.

If your content was removed and you believe this was a mistake or misidentification, you may send a counter-notice to [email protected] with identification of the removed material, your contact information, and a statement, under penalty of perjury, that you have a good-faith belief the material was removed by mistake.
